Navigating all the service providers in a community can be challenging, especially when in a state or crisis. With development of the Veteran Support Community initiative, a central point of contact - our Care Coordination Center - can direct individuals to core partners, and through them, to service providers in their local communities.
The Care Coordination Center receives and processes assistance requests from anywhere in the state. Our support center staff are peer support trained and can help navigate any issue a veteran or familiy members has. The also analyze trends in support requests to determine which areas are in greatest need.
